Tenant Maintenance Request Form

To be used for all NON-EMERGENCY repair needs.

For emergencies, call the Property Management Office:  215-2800.

Emergencies include serious leaks, failure of refrigerator, electrical wiring problems that present a fire hazard, roof leaks, air conditioning or heater failure in extreme weather, or any other problem that may be life threatening or cause serious property damage.

By making this request, tenant agrees that if work is determined to be unnecessary or is the result of tenant neglect, tenant will be responsible for any vendor expenses associated with the trip charge.  Tenants are responsible for all incidental repairs and cleaning as part of routine household maintenance.  When tenant does NOT allow key to be released for repairs, appointments will be scheduled for maintenance work.  Any tenant who is not present for an appointment that they agreed upon, will be responsible for the trip charge from the maintenance company.
